Apply for your Merchant Mariner Credential (MMC)

Follow the steps below to complete your medical certificate application

Application Process (Entry Level/Inland Vessels Only)

  • Step 1: Download and Fill out MMC Application form CG-719B

    • Download the MMC Application CG-719B and fill out with all information requested

      • Application Guide:​​

        • Section I: Fill out the personal information that is requested in the form ​

        • Section II: Check the box "Entry Level" under "Original" Only 

        • ​Section III:

          • Part 1: Skip this section unless you are exempt from a TWIC. (Most are not exempt)

          • Part 2 and 3: Check applicable boxes for criminal record and NDR.

        • Section IV

          • Part 1: Check yes or no for the outreach program. It is optional.

          • Part 5: Signature and Signature

          • Part 5b: Authorization of Oath (can be completed by Notary Public, Judge, or anyone allowed to administer an Oath)

          • Part 6: Skip unless requested by third party ​​

        • Print name at the bottom right of each page of application​​

  • Step 4: Submit the completed  application (CG-719B form) to the USCG National Maritime Center (NMC) online or by mail.

    • Online (preferred method): Email scanned application as a PDF to MMCApplications@uscg.mil.

      • Subject line of email: ​Last name, First name​

    • Mail: Send to the NMC address listed on the form.
